Parathyroid

Parathyroid adenoma localization with Tc-99m sestamibi — dual-phase early/delayed washout imaging and dual-isotope pertechnetate subtraction, as a living study template with a facility acquisition journey with Maggoo, interpretation cases, and board-style questions.

Preparation

Facts follow the protocol selected under Image acquisition (Dual-phase washout vs Dual-isotope subtraction, top-right toggle). Values are typical SNMMI teaching ranges.

Image acquisition

Top-down facility view. Maggoo follows the dual-phase washout protocol: check-in → IV Tc-99m sestamibi → early neck/chest imaging (10–15 min) → wait 1.5–3 h → delayed imaging → optional SPECT/CT → departure.

Image interpretation

Open-access clinical sestamibi parathyroid scintigrams (Radiopaedia, CC BY-NC-SA) for adenoma examples (including a two-patient early/delayed pairing), ectopic mediastinal adenoma, and multigland hyperplasia, with synthetic schematics for normal and artifact cards.

ABR practice questions

Board-style items on sestamibi mechanism/dosing, washout discordance, multigland/ectopic pitfalls, and the subtraction technique.
